The cost per case is as follows:
12 weeks -£830

Casework includes the following:

• allocation of a dedicated family support worker;
• an initial family meeting to ascertain the issues and establish a support plan;
• weekly home based sessions;
• weekly reports detailing work undertaken will be shared with the family and referrer;
• review meetings to assess progress and ascertain whether further support is required;
• minutes of family / review meetings will be provided to the family and referrer;
  signposting to additional services if necessary;
• management oversight / monitoring.

All information is managed in accordance with the Data Protection Act 1998 and the Caldicott principles.
